Case Note Entry Screen

Case Notes can be started and saved an unlimited number of times, once you are happy with the Note you must Mark as Finalized for the note to be saved into the Case Note Summary. 

Procedure: Follow the steps below to Select the Case for the Change Case Details screen.

Create a new Case Note

Procedure: Follow the steps below to add a new Case Note

Open the Client record and navigate to Case Notes in the Activity Menu.  A secondary Menu will appear select the Add Case Note Option

If the client has a Active Safety Concern you will see this message first

Press Review to see the details of each Safety Concern listed.

Press OK to continue with creating a New Case Note

Case Notes can be started and saved an unlimited number of times, once you are happy with the Note you must Mark as Finalized for the note to be saved into the Case Note Summary

The following fields must be completed before Finalizing the Notes

Mandatory Fields

  • Date of Activity – This is the actual Date of the appointment or activity.
  • Contact Type –– Select one type of Contact from the dropdown menu.

Non Mandatory Fields

  • Appointment Duration – This is the length of the appointment.
  • Travel Time: Time taken to travel to appointment ONLY.

Goals – This will display all active Goals for the Client selected.

If the ticked box is selected, the Case Note once finalized will link to the Goals and will display in the Case Note Summary, you will also be able to filter on the Goals selected.

Add/Edit Goals button this will open the Goals, where you can select a Goal to edit our Add New Goal.

Present – The first box is a list of Key Contacts, Relationships and Professionals that are linked to the client, but if there is someone present that is not on this list then type their name into the Free Text box

Staff Present –

This will display all active staff in alphabetical order.

  • Present – Tick the appropriate box if that staff member was present.

Review – If this is ticked an Alert will be sent to that staff member selected, with the title of “Review Case Notes for Client” (client name).

Future Appointments – Hyperlink to any appointments for the client selected, when selected the Appointment Details are available. This is turned off for Cholmondeley.

Notes – This is where you write your Case Note for the client selected. This fields expands as you required.

Mark as Important, when selected a small free text line will appear where it can be written, this will then appear on the Dashboard Widget for all staff to view while the selected child has a stay status of Onsite.

Confidential Notes – when selected the note will be confidential and only certain staff with access are able to view the note.

Save Button  At any stage you can click the Save Button to Save the Case Notes before Finalizing them.  However, you will need to go back and Finalize the note before they will be available in the Case Note Summary screen, you can select to view any unfinalized Case Notes from the Case Note Summary or go to Case Notes>Edit Case Notes   You will also get an Alert in your “My Tasks” Widget found in your Dashboard about any notes that have not been finalized after 2 days.  
Mark as Finalized Button  Click on the Mark as Finalized button, this will save and Finalize the Case Note and it will now be view only and cannot be changed.   
Add New ActionPress this button to enter a New Action, Please refer to the Activity Menu User Guide for full details on how to complete this screen.
Add Safety Concern button  Press this button to enter in a new Safety Concern. Please refer to the Activity Menu User Guide for full details on how to complete this screen.
Make Appointment button  Press this button to enter in an Appointment. Please refer to the Making Appointment User Guide for full details on how to complete this screen.
Review Case Summary Button  This button will take you to the Case Note Summary screen, where you can see all the finalised Notes for the client.
Upload Document button  If you wish to save a document against this client press this button, by default the Clients Name and Case will display in the Document Input Screen. Please refer to the Documents User Guide for full details on how to complete this screen. The document will display on the Case Note Summary screen as the Document Uploaded (Under the Review Outcome field and the above Future Appointment Field, and the document name is hyperlink and when you click on the hyperlink the document will open.  
Close Buttonwill close the screen you are on and return you to the previous screen you were in.
